this application that I served at least on day of active military duty after December 7, 1941

as indicated below, and was honorably discharged, or am still serving honorably.

Please provide a copy of your DD214 (with SSN blacked out) with your payment to verify eligibility.

NEW MEMBERSHIPS:  Please complete Part I and Part II of the Membership Application.

MEMBERSHIP RENEWAL:  Please complete Part I and Part III of the Membership Application.

​

MEMBERSHIP TRANSFER:  Please complete Part I and Part IV of the Membership Application.
